Matrix Medical Network is seeking a compassionate Nurse Practitioner to provide in-home health assessments and support quality patient care. In this role, you will conduct comprehensive evaluations, coordinate care plans, and collaborate with multidisciplinary teams to improve outcomes and reduce healthcare costs. Perform comprehensive in-home and community-based health assessments for health plan members, including detailed histories and physical examinations.
Identify, evaluate, and manage acute and chronic conditions within NP scope of practice, developing evidence-based care plans.
Review and reconcile medications, assess adherence and potential interactions, and provide patient counseling on safe and effective medication use.
Document all assessments, findings, and care plans in the electronic health record (EHR) accurately, thoroughly, and in a timely manner.
Collaborate with primary care providers, specialists, care managers, and health plans to coordinate care, close gaps, and support quality initiatives.
Educate patients and families on disease management, lifestyle modifications, preventive care, and available community resources.
Order and interpret appropriate diagnostic tests in accordance with clinical guidelines and organizational protocols, when permitted by scope and state regulations.
Participate in quality improvement and population health initiatives, providing feedback from field experience to enhance care models and workflows.
Maintain compliance with all regulatory, documentation, privacy, and safety standards while working independently in home and community settings.
Manage daily schedule and travel logistics, ensuring punctuality, productivity, and professional representation of Matrix Medical Network.
Advanced clinical assessment and diagnostic skills

Current, unrestricted Nurse Practitioner license in state of practice
National Board Certification as an NP (e.g., AANP or ANCC)

Valid driver’s license and reliable transportation
Experience with EHR systems preferred
