We are looking for a Full-Time provider (32-40 hours per week) for our Lebanon Center located in Lebanon, NH. This role is open to both nurse practitioners and physician assistants, and we encourage new graduates to apply.
The annual salary for this position ranges from $112,320 to $124,800, along with a no-cap incentive compensation bonus program.

Key ResponsibilitiesThe Advanced Practice Provider will report directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. This role involves collaborating on-site with the Center Leadership Team to meet specific center goals and priorities. Additionally, the Advanced Practice Provider will work alongside the Medical Director and/or Attending Physician to deliver patient-specific medical care.
The Advanced Practice Provider will offer primary and urgent care services to residents of the nursing center, working in conjunction with other medical providers. This role is essential in helping our organization fulfill its mission of enhancing the lives we impact through high-quality healthcare and compassionate service.
QualificationsThe ideal candidate must have graduated from an accredited School of Nursing and hold a valid registered nurse license. For Nurse Practitioners, certification is required from an accredited Nurse Practitioner Program with a Master’s Degree in Nursing, focusing on Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations may be considered based on experience and organizational needs. Physician Assistants must graduate from a program accredited by the ARC-PA and pass the PA National Certifying Exam (PANCE) to use the PAC designation. State licensure for PAs is also required, along with NCCPA certification.
A minimum of two years of experience working with adult or geriatric populations is preferred.
In states where practice regulations permit, APRNs must hold an unrestricted professional license and DEA registration for each state of practice. PAs must also have an unrestricted professional license and DEA registration for their respective states.

Note: Restrictions may apply based on collective bargaining agreements, state laws, and factors such as pay classification, job grade, location, and length of service. To qualify for full-time benefits, a minimum of 30 hours per week is required. Salary InformationSalary Range: $112,320.00 - $124,800.00 per year. Bonus: $5,000.00
