• Engage patients and their families in shared decision-making, offering clear explanations regarding diagnoses, medications, and lifestyle changes.
• Facilitate smooth transitions of care across different settings and specialties.
• Document all patient interactions accurately and promptly in the electronic health record system.
• Adhere to regulatory and compliance standards while participating in quality improvement initiatives.
• Contribute to clinical protocols and assist in implementing best practices to enhance care delivery.
