We are looking for a healthcare provider for a part-time role, requiring 32 hours per week, divided between two facilities located in Frederick, MD, and Walkersville, MD: Autumn Lake Healthcare at Ballenger Creek and Autumn Lake Healthcare at Glade Valley. This position is open to either a nurse practitioner or a physician assistant, with a minimum of one year of relevant experience required.
The hourly wage for this role ranges from $56.73 to $59.13, along with an incentive compensation bonus program that has no cap.

Key ResponsibilitiesThe Advanced Practice Provider will report to the Director of Clinical Practice and/or the Area Clinical Practice Manager. This role involves collaborating on-site with the Center Leadership Team to meet specific goals and priorities. Additionally, the Advanced Practice Provider will work alongside the Medical Director and/or Attending Physician to deliver patient-specific medical care.
The Advanced Practice Provider will offer primary and urgent care services to residents of the nursing center, working in conjunction with other medical professionals. This position is integral to achieving our mission of enhancing lives through high-quality healthcare and compassionate service.
QualificationsThe ideal candidate must have graduated from an accredited nursing program and hold a registered nurse license. For nurse practitioners, certification is required from an accredited Nurse Practitioner Program with a Master’s Degree in Nursing, focusing on Older Adult or Family Nurse Practitioner (FNP) specialties. Other concentrations may be considered based on experience and organizational needs. For physician assistants, graduation from an ARC-PA accredited program and successful completion of the PA National Certifying Exam (PANCE) is necessary, along with state licensure and NCCPA certification.
A minimum of two years of experience working with adult or geriatric populations is preferred.
In states where practice regulations permit, the Advanced Practice Provider must hold an unrestricted professional license and DEA for each state of practice.

Note: Restrictions may apply based on collective bargaining agreements, state laws, and factors such as pay classification, job grade, location, and length of service. To qualify for full-time benefits, a minimum of 30 hours per week is required. Some benefits, including health plans, may not be available to part-time employees working between 16 and 29 hours per week.
Salary RangeHourly wage: $56.73 - $59.13
