Matrix Medical Network seeks a Nurse Practitioner to deliver high-quality, in-home health assessments and care management services. Perform comprehensive in-home health assessments for diverse patient populations
Develop and manage individualized care plans to improve patient outcomes
Collaborate with interdisciplinary teams, payers, and community resources
Document findings accurately in EHR and support quality reporting requirements
Educate patients and caregivers on disease management and prevention
Identify gaps in care and coordinate appropriate follow-up services
Participate in quality improvement and innovation initiatives
Maintain compliance with clinical, regulatory, and safety standards
