Description
Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for Medicare and high-risk patients in their homes. In this field-based role, you will complete detailed assessments, reconcile medications, identify care gaps, and develop evidence-based care plans supported by our analytics and technology platform. You will collaborate with interdisciplinary teams to advance value-based care, improve outcomes, and reduce avoidable costs while enjoying a mission-driven, innovative, and flexible work environment focused on work–life balance and professional growth.Conduct comprehensive in-home health assessments for Medicare and high-risk patients.
Review medical histories, medications, and social determinants of health to identify care gaps.
Develop evidence-based care plans and document findings in electronic health records.
Collaborate with Signify Health clinical, care coordination, and analytics teams.
Educate patients and caregivers on disease management, medications, and preventive care.
Use data-driven insights and technology tools to support value-based care decisions.
Communicate findings to primary care providers and health plan partners as appropriate.
Follow clinical guidelines, quality metrics, and regulatory requirements for in-home care.
Maintain accurate, timely clinical documentation and coding for all encounters.
Contribute to continuous improvement initiatives and uphold Signify Health’s mission and values.
Advanced health assessment and physical examination
Chronic disease management (e.g., CHF, COPD, diabetes)
Care planning and care coordination
Electronic health record (EHR) documentation
In-home or community-based patient care
Medication reconciliation and management
Knowledge of value-based care and quality metrics
Patient and caregiver education
Telehealth and mobile health technology use
Clinical decision-making and diagnostic reasoning

Active, unrestricted Nurse Practitioner or Physician Assistant license in the state of practice
National board certification as an NP (e.g., ANCC, AANP) or PA (NCCPA)
Current DEA registration (or eligibility, as required)

Valid driver’s license and reliable transportation for in-home visits
