Description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to deliver comprehensive, value-based care in patients’ homes. In this role, you will perform detailed health evaluations, manage chronic conditions, reconcile medications, and identify care gaps for vulnerable populations. Using modern technology and data-driven insights, you’ll document in EHRs, collaborate with interdisciplinary teams, and coordinate follow-up services. Assess medical history, chronic conditions, medications, and social determinants of health.
Develop evidence-based care plans and collaborate with primary care providers and care teams.
Document findings accurately in electronic health records and Signify Health platforms.
Use data and analytics tools to identify care gaps and support value-based care goals.
Educate patients and caregivers on disease management, medications, and preventive care.
Coordinate follow-up services, referrals, and community resources as needed.
Adhere to clinical guidelines, regulatory standards, and Signify Health protocols.
Participate in interdisciplinary case discussions and quality improvement initiatives.
Leverage technology for scheduling, telehealth touchpoints, and secure communication.
Advanced physical assessment

Active, unrestricted Nurse Practitioner or Physician Assistant license in practicing state
National board certification (e.g., AANP/ANCC for NPs or NCCPA for PAs)
Current DEA registration (or eligibility) for prescribing as required
BLS certification; ACLS preferred
Valid driver’s license and reliable transportation for home visits
