Description

Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to deliver value-based care in patients’ homes. You will perform comprehensive health assessments, review medications and social factors, and create evidence-based care plans for vulnerable populations. Using Signify’s technology platform, you’ll document visits, close care gaps, and collaborate with interdisciplinary teams. Perform comprehensive in-home health assessments for diverse and often vulnerable patient populations.
Review medical histories, medications, and social determinants of health to identify risks and care gaps.
Develop and document evidence-based care plans and recommendations in alignment with value-based care goals.
Collaborate with physicians, care coordinators, and health plan partners to support continuity of care.
Use Signify Health’s technology platform for documentation, decision support, and quality reporting.
Educate patients and caregivers on conditions, medications, and preventive care to improve outcomes.
Ensure compliance with clinical guidelines, HIPAA, and regulatory requirements during all visits.
Contribute clinical insights to interdisciplinary teams to refine programs and population health strategies.
