Description

Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations in patient homes. You will conduct detailed histories and physicals, reconcile medications, identify care gaps, and develop evidence-based care plans that support value-based care. Using Signify’s technology platform, you’ll document visits, share insights with primary care teams, and help coordinate services for vulnerable populations. Conduct comprehensive in-home health assessments for adult and senior patients
Review medical histories, medications, and social determinants of health
Develop evidence-based care plans and communicate findings to primary care providers and health plans
Use Signify Health’s technology platform to document visits accurately and efficiently
Identify gaps in care and recommend preventive services and community resources
Collaborate with interdisciplinary teams to support value-based care goals
Educate patients and caregivers on chronic disease management and healthy lifestyles
Ensure compliance with clinical guidelines, quality standards, and regulatory requirements
Maintain patient safety, professionalism, and cultural sensitivity in all home visits
Participate in quality improvement initiatives and adopt data-driven best practices

National board certification as NP (e.g., AANP, ANCC) or PA (NCCPA)

Valid driver’s license, reliable transportation, and proof of insurance
Experience with home visits or community-based care preferred
