Description

As an In-Home Nurse Practitioner or Physician Assistant at Signify Health, you will perform comprehensive in-home assessments for diverse, often vulnerable patients. You’ll review histories, medications, and social factors; identify risks and care gaps; and create evidence-based care plans that support value-based care. Using Signify Health’s technology, you’ll document visits, collaborate with interdisciplinary teams, and educate patients and caregivers. This role combines autonomous practice, data-driven insights, and mission-focused work to improve outcomes where patients live.
Conduct comprehensive in-home health assessments for diverse and often vulnerable patient populations
Review medical histories, medications, and social determinants of health to identify risks and care gaps
Develop evidence-based care plans and communicate findings to primary care providers and health plans
Use Signify Health’s technology platform to document encounters and support data-driven decision-making
Educate patients and caregivers on condition management, preventive care, and available community resources
Collaborate with interdisciplinary teams to support value-based care initiatives and care coordination
Adhere to clinical quality, documentation, and regulatory standards for in-home evaluations
Participate in ongoing training, quality improvement, and performance feedback programs

National board certification as NP (e.g., AANP, ANCC) or PA-C (NCCPA)
Current DEA registration (or eligibility) for prescribing where required
BLS certification; ACLS preferred
Valid driver’s license and reliable transportation for in-home visits
Experience with EHR systems and mobile clinical documentation tools
