Description

Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to conduct comprehensive health evaluations for patients in their homes. You will review histories, perform focused exams, assess social determinants, and develop evidence-based care recommendations that support value-based care. Partnering with interdisciplinary teams, you’ll use modern technology, EMR systems, and data insights to close care gaps, coordinate services, and educate patients. Conduct comprehensive in-home health evaluations for diverse, often vulnerable, patient populations
Review medical history, medications, and social determinants of health to identify risks and care gaps
Perform focused physical exams, order or review diagnostic tests, and document findings in EMR systems
Develop evidence-based care recommendations and communicate them to patients, caregivers, and care teams
Collaborate with Signify Health’s interdisciplinary teams to support value-based care and care coordination
Use data and clinical guidelines to inform decision-making and improve quality and outcomes
Educate patients on chronic disease management, preventive care, and community resources
Maintain accurate, timely, and compliant clinical documentation
Leverage telehealth and mobile technology to support visit preparation and follow-up
Adhere to all regulatory, quality, and safety standards while working independently in the field

National board certification as an NP (e.g., FNP, AGNP) or PA-C

Valid driver’s license and reliable transportation for in-home visits
