Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations in patients’ homes. You will assess chronic conditions, medications, and social needs, document findings in EMR, and create actionable care plans that support value-based care. Partner with interdisciplinary teams, use data and modern technology to close care gaps, and educate patients and caregivers. Perform comprehensive in-home health evaluations for diverse, often vulnerable patient populations
Assess chronic conditions, medications, functional status, and social determinants of health
Document findings accurately in EMR and generate clear, actionable care recommendations
Collaborate with Signify Health’s interdisciplinary teams to support value-based care plans
Educate patients and caregivers on disease management, preventive care, and community resources
Use data and clinical insights to close care gaps and improve quality metrics
Maintain high standards of safety, confidentiality, and regulatory compliance during home visits
Leverage telehealth tools and modern technology to coordinate care and follow-up
