Description
As an In-Home Nurse Practitioner or Physician Assistant at Signify Health, you will perform comprehensive in-home evaluations for diverse and often vulnerable patients. You’ll assess chronic conditions, medications, and social determinants of health to close care gaps and inform value-based care. Using Signify’s technology and analytics, you’ll create evidence-based care plans, document thoroughly, and collaborate with interdisciplinary teams. This role offers autonomy, mission-driven work, and opportunities to innovate within a supportive, data-focused culture.Conduct comprehensive in-home health evaluations for diverse and vulnerable patient populations.
Assess chronic conditions, medications, functional status, and social determinants of health.
Develop and document individualized care plans using evidence-based guidelines and Signify Health tools.
Collaborate with interdisciplinary teams and communicate findings to primary care and care management partners.
Use Signify Health’s technology platform to capture accurate, data-driven clinical documentation.
Provide patient education on disease management, preventive care, and appropriate follow-up.
Identify care gaps, risk factors, and community resources to support value-based care goals.
Maintain high standards of quality, compliance, and patient safety in all home visits.
Contribute to continuous improvement initiatives and adopt new workflows and analytics tools.

Advanced health assessment and physical examination
Chronic disease management (e.g., diabetes, CHF, COPD)

Active, unrestricted Nurse Practitioner or Physician Assistant license in applicable state(s)
National board certification as an NP (e.g., ANCC, AANP) or PA (NCCPA)
Current DEA registration (or eligibility) for prescriptive authority as required
BLS certification; ACLS preferred
Valid driver’s license, reliable transportation, and ability to travel to patient homes
Experience with Medicare Advantage, risk adjustment, or value-based care preferred
