Description

As an In-Home Nurse Practitioner or Physician Assistant at Signify Health, you will deliver comprehensive in-home evaluations and manage acute and chronic conditions for vulnerable populations. Using modern technology and data-driven insights, you’ll document visits in EHRs, create patient-centered care plans, and collaborate with interdisciplinary teams to advance value-based care. You’ll identify clinical and social needs, coordinate resources, and educate patients and families while working in a mission-driven, innovative, and supportive culture focused on improving outcomes.
Conduct comprehensive in-home health evaluations for diverse and vulnerable patient populations.
Assess, diagnose, and manage acute and chronic conditions following evidence-based guidelines.
Document encounters accurately in electronic health records and leverage data-driven insights.
Collaborate with physicians, care coordinators, and health plan partners to support value-based care.
Develop and update patient-centered care plans and provide education on treatment and prevention.
Identify social determinants of health and coordinate with community resources when appropriate.
Follow all regulatory, quality, and safety standards and maintain clinical best practices.
Use mobile/telehealth tools and modern technology platforms to support in-home visits.

Valid driver’s license and reliable transportation for in-home visits
