Description

Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for vulnerable patients in their homes. You will review histories, medications, and social factors, document in EMRs, and identify care gaps to support value-based care. Collaborating with interdisciplinary teams, you’ll leverage technology and data to guide decisions, educate patients and caregivers, and improve outcomes. Conduct comprehensive in-home health assessments for Medicare and vulnerable populations.
Review medical histories, medications, and social determinants of health to identify care gaps.
Document findings accurately in EMR and provide evidence-based recommendations.
Collaborate with Signify Health’s interdisciplinary teams to support value-based care plans.
Educate patients and caregivers on conditions, medications, and preventive care.
Use telehealth tools and data-driven insights to inform clinical decision-making.
Follow quality, compliance, and safety standards for home visits.
Participate in ongoing training, quality improvement, and performance metrics review.
