Description
Element Care seeks an Advanced Practice Provider (NP/PA) to deliver compassionate, high-quality care to older adults in our PACE program. In this role, you will provide primary and preventative care, manage chronic and acute conditions, and complete comprehensive geriatric assessments across clinic, home, and community settings. You will collaborate closely with an interdisciplinary team to build individualized care plans that support seniors to live safely and comfortably at home. This position emphasizes holistic, person-centered care, strong care coordination, and meaningful relationships with participants and caregivers, directly improving quality of life for elders in your community.Provide comprehensive primary and preventative care to PACE participants in clinic, home, and community settings.
Assess, diagnose, and manage acute and chronic conditions for older adults.
Develop, implement, and adjust individualized care plans in collaboration with the interdisciplinary team.
Prescribe and manage medications, diagnostics, and treatments according to clinical guidelines.
Conduct routine geriatric assessments, including functional, cognitive, and psychosocial evaluations.
Educate participants and caregivers on disease management, wellness, and safe aging at home.
Participate in interdisciplinary team meetings to coordinate medical, social, and supportive services.
Document all care in the electronic health record in a timely, accurate manner.
Support transitions of care, including hospitalizations and post-acute follow-up.

Adult/geriatric primary care
Chronic disease management (e.g., diabetes, CHF, COPD)
Comprehensive geriatric assessment

Electronic health records (EHR) documentation
Prescribing and medication management
Wound care and functional assessments
Interdisciplinary team collaboration
Patient and family education
Regulatory and compliance knowledge in PACE or similar programs

Valid Nurse Practitioner (NP) or Physician Assistant (PA) license
National board certification (e.g., AANP, ANCC, NCCPA)
BLS certification
DEA registration (or eligibility)
