Description
OnPoint Medical Group is seeking a compassionate Nurse Practitioner or Physician Assistant to join our physician-led primary care team in Healthcare & Medical Services. In this collaborative, patient-first environment, you will provide comprehensive primary care, including assessment, diagnosis, treatment, and chronic disease management for patients across the lifespan. Responsibilities include conducting exams, ordering and interpreting tests, prescribing medications, and delivering preventive and wellness care. You will work closely with physicians, behavioral health, and ancillary services to coordinate care, support quality initiatives, and build long-term relationships within local Denver metro communities. Conduct physical exams, order and interpret diagnostic tests, and develop evidence-based care plans
Prescribe medications and manage chronic conditions in collaboration with supervising physicians
Deliver patient education, preventive care, and wellness counseling to support long-term health
Document all encounters accurately in the electronic health record and maintain regulatory compliance
Collaborate closely with physicians, nurses, behavioral health, and ancillary teams to coordinate patient-centered care
Participate in quality improvement initiatives and adhere to clinical protocols and best practices
Support care transitions, follow-up visits, and care coordination within the Denver metro community
Contribute to a respectful, team-based culture that emphasizes communication and continuous learning
Engage in professional development to maintain licensure, certifications, and clinical competencies
Primary care clinical assessment and diagnosis
Chronic disease management (e.g., diabetes, hypertension)
Electronic health record (EHR) documentation
Ordering and interpreting labs and imaging
Evidence-based treatment planning
Medication management and prescribing
Preventive care and wellness counseling
Care coordination and population health
Patient education and communication
Clinical triage and acute care management

Active Nurse Practitioner (NP) or Physician Assistant (PA-C) license
Current state licensure in practicing state
National certification as NP (AANP/ANCC) or PA (NCCPA)
Current DEA registration for prescribing
BLS certification; ACLS preferred
Experience in primary care, family medicine, or internal medicine
