Description
Westchester Institute for Human Development seeks a compassionate Nurse Practitioner to provide comprehensive, person-centered primary and specialty care to children and adults with developmental disabilities and vulnerable populations. Working within our Healthcare & Medical Services department, you will conduct assessments, manage chronic and complex conditions, prescribe treatments, and educate patients and families. Collaborate closely with an interdisciplinary team in our inclusive, mission-driven environment, contributing to clinical care, education, research, and advocacy that positively impacts the Hudson Valley community.Provide comprehensive assessments, diagnosis, and treatment for children and adults with developmental disabilities and vulnerable populations.
Manage acute, chronic, and complex medical conditions, including prescribing medications and ordering/interpreting diagnostics.
Develop and implement individualized, person-centered care plans in partnership with patients, families, and caregivers.
Collaborate with an interdisciplinary team of physicians, therapists, educators, and social workers.
Educate patients, families, and community partners on health promotion, disease management, and available resources.
Document care in the EHR, ensuring accurate, timely, and compliant records.
Participate in quality improvement, research, and educational initiatives that advance best practices in disability-focused healthcare.
Contribute to WIHD’s advocacy and community outreach efforts in the Hudson Valley region.
