CNY Family Care, LLP is seeking a dedicated and experienced Nurse Practitioner to join our team, providing comprehensive healthcare services tailored to family needs from pediatric to geriatric care, including specialized services like gynecology and acute care. This position avails an opportunity to engage deeply with patients in East Syracuse, offering personalized and effective healthcare solutions as part of a collaborative multidisciplinary team.
Conduct thorough patient exams and assess health history
Develop and implement patient management plans, record progress notes, and assist in provision of continuity of care
Prescribe medications following set guidelines and regulations
Order, perform, and interpret diagnostic tests
Collaborate with multidisciplinary healthcare teams to provide patient care
Educate and guide patients on disease prevention and healthy lifestyle habits
Manage overall patient care regarding lifestyle and development issues
Respond to patients' clinical needs promptly
Strong clinical skills with ability to perform comprehensive and problem-focused histories and physical examinations
Excellent knowledge of clinical practices and guidelines
Ability to maintain detailed and accurate records
Strong decision making and problem-solving skills
Outstanding communication and interpersonal abilities
Proficiency in using healthcare software and systems
