Matrix Medical Network seeks a Nurse Practitioner to deliver high-quality in-home and community-based health assessments for diverse adult populations. In this role, you will perform comprehensive exams, review medical histories, manage chronic conditions, and develop evidence-based care plans that enhance patient outcomes and support value-based care. You’ll collaborate closely with an interdisciplinary team, leverage telehealth and digital tools, and ensure accurate EHR documentation and regulatory compliance. Conduct in-home and community-based health assessments for diverse adult populations
Evaluate medical history, perform physical exams, and develop evidence-based care plans
Collaborate with interdisciplinary teams to optimize patient outcomes and close care gaps
Document findings accurately in EHR and support quality and compliance initiatives
Educate patients and caregivers on disease management, medications, and preventive care
Use telehealth and digital tools to monitor health status and coordinate follow-up care
Participate in quality improvement projects that support value-based care and cost reduction
Advanced patient assessment
