Wesley Group seeks a Concierge Nurse Practitioner to deliver high-touch, data-informed care across home, virtual, and clinic settings. In this role, you’ll manage a dedicated patient panel, providing comprehensive assessments, treatment, and care coordination while leveraging telehealth and digital tools. You’ll collaborate closely with physicians and Wesley Group leaders to design and refine innovative care models, track outcomes, and elevate patient experience. Provide concierge-style primary and episodic care to a defined patient panel in home, virtual, and clinic settings
Conduct comprehensive assessments, diagnose conditions, and develop evidence-based treatment plans
Coordinate care with physicians, specialists, and community resources to ensure seamless patient experience
Leverage telehealth and digital tools to deliver timely, data-driven care and follow-up
Document all encounters in EHR system, tracking outcomes, quality metrics, and patient satisfaction
Educate patients and families on conditions, medications, and preventive care strategies
Participate in quality improvement initiatives and help refine concierge care protocols
Collaborate with Wesley Group leaders to pilot innovative care models and patient programs
Advanced clinical assessment
